The History Since the 1950s, American Indian and Alaska Native communities have suffered a drastic increase in diabetes due to rapid transitions in both lifestyle and environment. American Indians and Alaska Natives are more than two times more likely to have diabetes than the general population of the United States.
The Goal The purpose of the Diabetes Wellness Program at the Indian Walk-In Center is to create awareness, encourage prevention and improve the on-going health care of American Indian and Alaska Native individuals and families living with diabetes. We assist people in leading a healthier lifestyle, including at-risk individuals who have not yet been diagnosed, through an assessment to determine diabetes risk factors -- an important first step in preventing or identifying the symptoms of this disease.
The Services The Diabetes Wellness Program offers education for prevention; proper nutrition, physical exercise, intervention services, and direct referrals, medical screening and care:
The Diabetes Wellness Program focuses on elevating the health status of American Indian and Alaska Natives to the highest level possible through lifestyle modification and preventative health strategies. Our client services are provided at no charge for American Indians and Alaska Natives living in the Wasatch Front area. An appointment is necessary for services that are provided at the Indian Walk-In Center.

Health Promotion and Disease Prevention
To register for Health Services: 1. Call the Health Referral Specialist for an appointment to discuss your health needs.2. Bring a Photo ID to your appointment 3. Bring documentation of income 4. Bring proof of residency (gas, light, or rent receipt with this month's date on front) 5. Bring Social Security Numbers for yourself and family members 6. Bring documentation of Indian Blood 7. Know your type of health insurance (Medicaid, CHIP, Utah Primary Care Network or other provider) The Healthcare referral program hours are as follows
First time clients, M-F 8:30 to 11:30 AM Returning clients, M,T,W, and F, 1:30 to 4:00PM
HIV and Sexually Transmitted Infections (STI) Services. The HIV/STI Program provides free confidential counseling and testing for the following:
