Client Services
We are pleased to offer health care for American Indians and Alaska Natives along the Wasatch Front. Several services are provided through Community Health Centers, Inc. and include medical, dental, and prenatal case management services. There are three clinics across the Salt Lake Valley.

Health Care
The Indian Walk-In Center can help you gain access to any of the following services at Community Health Centers, Inc.:

  • Primary Health Care
  • Dental Services
  • Acute or Chronic Health Care
  • Immunizations
  • Prenatal Case Management
  • Eye Care Services
  • Nutrition Counseling
Clinical care is available during daytime, evening, and Saturday hours.

Health Promotion and Disease Prevention
Health Promotion and Disease Prevention is an important part of our Health program and includes the following:

  • Annual Health Fair
  • Health Presentations
  • Client Eligibility Assistance for Medicaid, Medicare, Children's Health Insurance Plan and Utah's Primary Care Network
To register for Health Services:
  1. Call the Health Referral Specialist for an appointment to discuss your health needs.
  2. Bring a Photo ID to your appointment
  3. Bring documentation of income
  4. Bring proof of residency (gas, light, or rent receipt with this month’s date on front)
  5. Bring Social Security Numbers for yourself and family members
  6. Bring documentation of Indian Blood
  7. Know your type of health insurance (Medicaid, CHIP, Utah Primary Care Network or other provider)

    The Healthcare referral program hours are as follows

    First time clients, M-F 8:30 to 11:30 AM

    Returning clients, M,T,W, and F, 1:30 to 4:00PM

    HIV and Sexually Transmitted Infections (STI) Services
    The HIV/STI Program provides free confidential counseling and testing for the following;

    1. HIV
    2. Chlamydia
    3. Gonorrhea and
    4. Referrals for Hepatitis vaccinations and screenings are available.
